Percutaneous Transhepatic Cholangiography

Abstract
The first radiologic demonstration of the intrahepatic biliary tree by the percutaneous transhepatic method was done by Huard and- Do-Xuan-Hop in 1937. Carter and Saypol renewed interest in percutaneous transhepatic cholangiography (PTC) and established it as a valuable diagnostic procedure in 1952. Since then, numerous studies have been reported which emphasized the results, complications, and hazards of PTC. In National Medical Center, conventional method of PTC was done in 50 cases from 1963 to 1965. Because of the serious complications, this procedure was not adopted since 66.0 hto and Tsuchiya modified this procedure using a long, thin needle in 1969. This modified method has the least complications. In National Medical Center, this modified method was adopted in Department of General Surgery and Radiology since November 1974. We have done PTC in 33 cases during the four months period showing the better visualization rate and less complications.
1. The procedure is very simple and easy.
2. Very high percentage of the visualization is obtainable if properly done.
3. Frequency and degree of the complication is considerably low comparing with that of the conventional method.
4. Accurate differentiation between obstructive jaundice and hepatocellular. jaundice is easily obtainable.
5. Shortening of the diagnostic period is possible.
6. Better surgical planning is possible through.
1) Confirming the nature of the obstruction such as location and number of stone or level of malignant obstruction.
2) Confirming the variation of the cystic duct.
3) Surgical method may be selected according to the degree of the common bile duct dilatation.
4) Common bile duct exploration can be determined by the F.T.C. finding.
7. Percutaneous transhepatic cholangiography is significant as the preoperative routine diagnostic method.
